What happens to your data?
Eligible personal account information and data that are no longer needed will be removed following verification. Patient records, shared hospital or business records, financial transactions and audit records may need to be retained for legal, clinical, security or accounting reasons. We will explain retained categories, reasons and the applicable period when reviewing your request.
Your email and request details are used to process and document this request. Limited anti-abuse information protects this form. Request records are retained only as needed for handling and accountability. Submitting a request does not immediately delete an account or workspace. Read our privacy policies.
